This is the "Incident" template used with Cargo and the Wiki extension.

Fields available: Company: (Optional) Related company pages; StartDate: (Optional) Start date of incident; EndDate: (Optional) End date of incident; Status: (Optional) Status of the incident (Active, Pending Resolution, Resolved); ProductLine: (Optional) Related product line pages; Product: (Optional) Related product pages; ArticleType (Optional) Indicates whether or not the incident is related to a "Product" or "Service"; Type: (Optional) Type of incident (e.g. Repairability, Privacy, etc.); Description: (Optional) Description of the incident in 150 characters or less
